1. Print and fill out the account blocking application, filling in the blank fields by hand. The application is attached to this email.

2. Attach a photo of the open passport page so that part of the passport touches the application.

3. Photograph all documents and send them in a reply email.

So they, in their infinite wisdom, sent a form that I’m supposed to print out and fill in, then scan, photograph and send back again.
